To access services through the Urban Indigenous Wellness Program, you'll need to complete the UIC membership enrollment and program intake
– This helps us understand your connection to community and ensures culturally appropriate care.
Select the Indigenous Survivors Program or LGBTQ2S+ Wellness Program based on your needs and interests.
After submitting your forms, a member of our Wellness Team will follow up to schedule an initial conversation and connect you with available services.
Please note: Spaces are limited for both programs at this time, and a short waitlist may apply.
