Community Health Needs Assessment

UIC is conducting  a community health needs assessment to gain a better understanding of the behavioral and mental health services that are available for AI/AN and Indigenous people in the Tri-State area, as well as their priorities for health services. 

Participation will take about 10 minutes. The survey is anonymous.
